Record Sharing and Care Data

From 2nd October 2013 the way we share your records is changing.

Your patient record is stored electronically on a system called TPP System One. If you are referred to other NHS professionals for example the District nurse, the practice intends to share your record with this health professional UNLESS you ask us not to.

The poster and patient leaflet explains in detail, please read and please inform us if you DO NOT want this to happen.
