To register as a patient you need to live within the practice area and our reception staff can show you a detailed map if you need to check.
Please complete your medical card or form GMS1 from the link below or available from reception. Please bring this form in along with a completed New Patient Registration Questionnaire in addition to a copy of the required ID verification documents mentioned in the Questionnaire. You may be asked to attend a new patient consultation with a member of our team which may include a Clinical Practitioner or a Doctor. This will provide us with information needed whilst we wait for your medical records to arrive.
Please note you will not be registered at the practice until you have come into the surgery and have been seen by the Nurse and Doctor.
