INTRODUCTION

We produced our first Practice Formulary in 1993. Three formal versions and some intermediate changes later, we have now produced the first draft of an Evidence-Based Formulary (EBF). This document lists medications we believe should be initiated or maintained by our Practice.

We have not included medications initiated and managed either in Secondary Care, or in Primary Care under formal, or informal, Shared Care arrangements. We believe that a minimalist approach needs only to be tempered by a need to produce comprehensive coverage of BNF sections and sub-sections. We have subjected the list of recommended drugs to a process of consultation over the last year. We hope that as a result it should accurately reflect much of our prescribing for some time to come. We include evidence and/or guidance that underpins the EBF where possible. We hope that this will help established as well as newer members of the Practice, and locums.

We believe that justification of prescribing decisions is equally important as the prescribing itself. There is still plenty of scope for improvement on the evidence-base, and updates to the information will continue to be posted to the EBF. We welcome further information from anyone using this Formulary: if you would like to contribute please e-mail us via james.larcombe@gp-A83054.nhs.uk. We will upgrade the EBF regularly: minor amendments may be added at any time; intermediate annual reviews are recommended, and at least every five years a new Edition should be published.
